Comments:
An unannounced renewal inspection was conducted from 1:30 - 2:15pm with the provider. There were six children present, ranging from 20 months to four years of age, for a total of 13 points. Children were observed during rest time and one child was observed napping. Two child records, four applicant/household member records, the emergency drill log, written daily attendance, and menu were reviewed. It was reported that there were no medications on site, and the provider does not have current certification in Medication Administration Training (MAT) so cannot administer medication to children in care.

Based on review of four applicant/household member records, it was determined that not all household members had an original central registry report, within 30 days of becoming 14 years old, as required. Evidence: the record for household member #4 (date of birth: 12/29/2009) did not contain documentation of a central registry search report.
Plan of Correction:
Will request a central registry report.
