Comments:
An unannounced, on-site monitoring inspection was initiated on 7/8/2024 and completed on 7/8/2024, as a part of the licensure period. The on-site inspection began at 11:04AM and ended at 1:00PM. The inspector reviewed compliance in the areas listed above. There were 103 children, 16 staff and 2 administrative staff present. The inspector reviewed 4 children?s records, 5 staff records and 10 medications on-site. This inspection included-
? document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), ? tour of the facility, ? interviews, ? observations (this would include observations of photos, recordings, etc.)
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (date). A POC submitted after this date will not appear on the public website.
Areas of the center, inside and outside, shall be maintained in a safe and operable condition. The bottom right hinge was not attached to the cabinet door on the right above the sink in the 2 ? year old room. The bottom right corner of the cabinet door would pull away from the cabinet when opened.
Plan of Correction:
Provider will repair hinge on cabinet door.
