A Subsidy Health and Safety inspection was conducted on 11/18/19 to review supplemental health and safety requirements. There were 30 children present with nine staff. Five children's records were reviewed during this inspection. Five staff records were reviewed at separate site visit. This center does not administer medications. The inspector arrived at the center at 9:15 AM and departed at 1:30 PM. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Based on record review, the center failed to ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence:
1. Staff 1 had a documented start date of 09/18/2018 and a documented TB evaluation date of 10/02/2018. 2. Staff 2 had a documented start date of 02/02/2019 and a documented TB evaluation date of 02/12/2019.
Plan of Correction:
Staff will ensure that TB screenings/tests are finished prior to hire.
