Comments:
An unannounced monitoring inspection was conducted from 12:30pm-2:15pm. There were 49 children in the direct care and supervision of 9 staff members. A sample size of 5 children's records, 4 staff records,4 emergency medications, evacuation drills, the first aid kit, emergency materials, the playground, and the physical plant were reviewed. The children were observed eating lunch, being read books, coloring a map, completing puzzles, participating in circle time, using the restroom, and taking a nap. Based on a review of children's records, 1 out of 3 children's records was not complete. Evidence: 1. Child #2 was missing 2 of 2 required addresses for emergency contacts.
