This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 05/28/2020 and concluded on 05/29/2020. The director was contacted by telephone to initiate the inspection. There were 68 children present and nine staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed four children's records and four staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on a review of four children's records and interview, the center did not ensure that one child had a physical examination by or under the direction of a physician before the child's attendance; or within one month after attendance.
Evidence: 1) The physical examination in the record for Child #4, enrolled on 06/17/2019, was dated 05/14/2020. 2) During interview, a member of management confirmed there was no physical examination on file for Child #4 prior to 05/14/2020, which was received by the center on 05/28/2020.
Plan of Correction:
The physical examination was requested from the parent and submitted on 05/28/2020.
