This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 06/02/2021 and concluded on 06/18/2021. The director was contacted by telephone to initiate the inspection. There were 76 children present and 17 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.
Based on a review of four staff records and interview, the vendor did not ensure three staff who work directly with children completed the department's health and safety update course annually.
Evidence: 1) The most recent Annual Health and Safety Update Training in the record for Staff #2, hired on 08/19/19, was completed on 08/15/19. The training was not completed annually in 2020.
2) The most recent Annual Health and Safety Update Training in the record for Staff #3, hired on 11/09/18, was completed on 04/19/19. The training was not completed annually in 2020 or 2021.
3) The most recent Annual Health and Safety Update Training in the record for Staff #4, hired on 10/30/19, was completed on 10/30/19. The training was not completed annually in 2020.
4) During interview, a member of management acknowledged Staff #2, Staff #3, and Staff #4 did not complete the Annual Health and Safety Training annually.
Plan of Correction:
Per the Vendor: "All staff have completed the course and we will continue do it yearly. The director will check these trainings yearly."
