Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 01/11/2021 and concluded on 01/13/2021. The director was contacted by telephone to initiate the inspection. There were 71 children present and 17 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review, children's records did not contain all required information. Evidence: 1) The record for Child #3 did not contain a work phone number for one of the custodial parents. 2) The record for Child #4 did not contain a work phone number for each custodial parent.

Based on record review, the center did not ensure to request, at least annually, parent confirmation that the required information in the child's record is up to date. Evidence: 1) The most recent review of the information in the record for Child #3 was dated 09/13/2019.
