Comments:
An unannounced monitoring inspection was conducted today from 8:30 am-10:30 am. There were 43 children (infants-5yrs.) directly supervised by 9 staff. The physical plant, staff records, children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in parent drop off, group play, reading books, circle time and eating snack. Diapering and hand-washing procedures were also observed. There was an ample supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Based on review of records, one staff record did not contain documentation of a completed Sworn Disclosure (SDS) prior to hire.
Evidence: Staff A - record contained a completed SDS that expired 4/2023.

Evidence:
Staff in toddler classroom did not wash hands before and after changing diaper of child.

We will remind substitutes that hands need to be washed after each diaper change.

Description:
Based on observation, a nonabsorbent surface for diapering was not used.
Evidence: The diaper changing pad in the toddler classroom had a tear, making it no longer nonabsorbent.
Plan of Correction:
Remind substitutes that the diaper changing pad needs to be cleaned and sanitized after each diaper change.
