An unannounced Code compliance inspection was conducted today, 11/6/19. Upon arrival to the center the children were taking a nap. There are 15 children in care with two staff in the classroom and one staff conducting parent/teacher conferences. A review of staff and children's records was also conducted during today's inspection. Should you have any questions please do not hesitate to contact me. Based on review of staff records the center failed to have a staff complete a sworn disclosure statement and a central registry check within the set time frames. Evidence: 1. Staff #1, 3 & 4 did not have a sworn disclosure statement on file prior to the first day of employment. 2. Staff #2 & 6 have a hire date of 8/21/17 and there are no central registry record check on file.

The director with the assistance of the principal will ensure these forms are completed.
