Comments:
An unannounced monitoring inspection was conducted on January 28, 2020 to review subsidy health and safety inspection requirements. Provider was present with three children for a point total of ten. Provider and three children records were reviewed.
Based on review, provider did not ensure caregivers undergo TB screenings at least every two years from the date of initial screening.
Evidence: The most recent TB test/screening for provider was dated February 3, 2017 which is more than two years old.
Plan of Correction:
Provider will go to the health department to have the test or screening.

Evidence: There was documentation of two hours of training completed in 2019. Provider stated no other trainings were completed in 2019.
