Comments:
An unannounced monitoring inspection was conducted on January 28, 2020 from approximately 8:20 am to 10:30 am to review voluntary registration requirements. Provider was present with three children. Provider and three children records were reviewed.
Based on review, provider did not ensure 2 of 3 children records have documentation of signed statement from the parent affirming receipt of the information to parents statement.
Evidence: The records for child #1 (start date July 5, 2019) and child #2 (start date July 5, 2019) did not have a signed information for parents statement.
Plan of Correction:
Provider will have the parents signed the statement.

Description:
Based on observation and review, provider did not ensure the home met the criteria on the health and safety checklist at the time of the monitoring visit.
Evidence: The following was found during the inspection: 1. Emergency evacuation drills have not been practiced since September 2019 and the evacuation plan is not posted. 2. One household dog has a rabies vaccination that expired September 30, 2019.
Plan of Correction:
1. Provider will do monthly evacuation drills. 2. Provider will go the health department to have the test or screening done.
