Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated and concluded on December 10th, 2020. The Director was contacted by telephone to initiate the inspection. There were 66 children present with 16 staff. In Inspector emailed the Director a list of items required to complete the inspection. The Inspector reviewed 4 children's records and 4 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice. Based on a review of children's records, the center failed to obtain documentation that each child has received the immunization required by the State Board of Health before the child can attend the center.
Evidence: Child #2's record did not have documentation of immunizations on the first day of attendance. The child's first day of attendance was 08/24/2020, but documentation of immunizations was not dated until 08/25/2020.
Plan of Correction:
Documentation of immunizations received 08/25/2020 and placed in child's record.
