Technical Assistance:
Technical assistance was offered regarding background checks for employees and household members.
Comments:
An unannounced inspection was conducted in response to a complaint received on 1/6/20. The inspection was conducted from 12:15 pm to 1:10 pm. There were 12 children (28 points) in care of 4 caregivers. The complaint was related to persons residing in the family day home without obtaining the required fingerprint results. 4 staff were interviewed and a walk through of the family day home was conducted. 4 caregiver records and 1 household member record was reviewed. Based on the preponderance of evidence, the complaint is not valid. Violations non-related to the complaint are listed in the Violation Notice. Based on records review, the family day home did not obtain the sworn disclosure statement (SDS) by the first day of employment. Evidence: 1. Caregiver # 1's first day of employment was 1/21/20. 2. Caregiver # 1's record did not have a complete SDS.
Plan of Correction:
I will have the caregiver complete the form today and keep in here record.
