Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.
A monitoring inspection was initiated on 1/10/2022 and concluded on 01/13/2022. The provider was contacted by telephone and a virtual inspection was conducted. There were 10 children present, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review and interview with the provider, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1) The records for Child #1 (start date 10/20/2021) and Child #2 (start date 09/07/2021) did not contain documentation of a physical examination report. 2) The provider could not locate the physical examination report in the records for Child #1 and #2.
Plan of Correction:
The provider shall contact the parents of Child #1 and Child #2 and request a physical examination report.
