Technical Assistance:
Clarification was offered on the following topics and documentation emailed to the provider, when applicable: *Correct sworn statement form to be used *Medication authorization forms and log *Outside play area and equipment
Comments:
A renewal inspection was conducted on 08/14/2023 between the hours of approximately 2:00 pm and 3:30 pm. There were 6 children present, ranging in ages from 5 months to 4 years, with 4 staff supervising. Children were napping at the time of inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the provider did not ensure to obtain a fingerprint-based criminal history check determination letter every five years for caregivers. Evidence: 1) The most recent fingerprint-based criminal history check determination letter in the record for Staff #2 was dated 02/16/2018. 2) The most recent fingerprint-based criminal history check determination letter in the record for Staff #3 was dated 03/05/2018.
Plan of Correction:
Completed the fingerprint process. Results came back on 08/17/2023.
