Comments:
An unannounced monitoring inspection was conducted on May 4, 2023. There were 9 children (21 points) with 2 caregivers. The home, children's records, caregiver records, household member records, menu's, evacuation logs, attendance logs, cribs, and outside play areas were reviewed. The children were observed finishing breakfast, feeding the fish, diaper changing, hand washing, story time and outside play. There were no medications on site. There are a variety of educational equipment and games. The family day home has a new approved curriculum and seems to create a very inviting space. Based on a review of children's records, the provider failed to obtain al required documents for all children enrolled.
Evidence: Child #4 and #5's records do not contain documentation of the names of persons other than the custodial parent's who are authorized to pick up the child.
Plan of Correction:
I will speak to the parents; however, the families are new to the area and have not authorized anyone to pick up the children.

Description:
Based on observation the areas and furnishings of the family day home, inside shall be maintained in a clean, safe and operable condition.
Evidence: The kitchen and dining area were cluttered.
