Technical Assistance:
Discussed that the provider's MAT certification is expired. Recommended changing medication policy to "does not administer' until a new certification can be obtained.
Comments:
An unannounced renewal inspection was conducted today from 10:50am-11:40am. There were 9 children directly supervised by 3 caregivers. The home, 3 caregiver records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and doing a worksheet. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Based on record review and provider statement, caregivers were allowed employment without the results of a national criminal record check. Evidence:. Caregiver 2 was caring for children on the date of inspection without the results of a criminal record check. Caregiver 2 was hired the week of March 4, 2024 and has been providing care since then.
Plan of Correction:
National criminal record check will be provided prior to any new provider enrollment. Caregivers2 inspection has arrived 3/14/24 and has been sent two inspector.

REPEAT VIOLATION Based on record review, the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: 1. Caregiver 1's most recent Tuberculosis screening was conducted on 10/29/2021. 2. Caregiver 3's most recent Tuberculosis screening was conducted on 2/3/2020.
