Comments:
An unannounced monitoring inspection was conducted today from 8:55 am-10:15 am. There were 7 children directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, practicing tummy time, and drinking a bottle. Diapering procedures were also observed. There was an ample supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Based on record review, required child information was found to be incomplete. Evidence: Child #1- record is missing 2 of 2 emergency contact names, addresses Child #2- record is missing emergency contact addresses, parents work addresses and telephone numbers and authorization for emergency medical treatment.
Plan of Correction:
I will get from parents today.
