Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.
A monitoring inspection was conducted on 9/13/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present today, ranging in ages from 3 years to 4 years, with the provider. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records, 1 staff record, and 2 household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and provider interview, the provider did not obtain updated tuberculosis screening or testing results within the last two years for all caregivers and adult household members.
Evidence: Staff #1, Adult Household Member #1, and Adult Household Member #2 all had tuberculosis test results on file completed 1/15/2019.
