Comments:
An unannounced focused monitoring inspection was conducted 12/1/2022 from 9:15am through 11:05am. The inspection was conducted for the purpose of determining compliance in the areas that led to the center being placed on a 6 month Provisional License. There were a total of 32 children present with 9 staff and all classrooms were in ratio. The physical plant, outdoor playground area, 4 children's records and 2 staff records were inspected. Children were observed tracing numbers and making various holiday crafts. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.035-B-4
Description: Based on record review, the center did not conduct a follow up inquiry on an out of state child abuse and neglect search within 45 days after request to search for all staff who have lived outside the state of Virginia within the last 5 years.

Evidence: The center submitted a Maryland Child Abuse and Neglect Central Registry search for Staff #1 (Date of Hire: 5/11/2022) on 5/11/2022, however, no follow up was completed within 45 days after request to search.

Plan of Correction: Emailed and resent duplicate request.

Standard #: 8VAC20-780-500-B
Description: Based on observation, disposable diapers were not disposed in a way that prevents the staff member's hand from touching the exterior surface of the storage system during disposal.

Evidence: In the Infant room, Staff #1 used their hand to lift the lid of the trash can to dispose of their gloves and an infant's diaper instead of using the foot operated mechanism.

Plan of Correction: Remind teachers to use foot pedal.
