Comments:
A Subsidy Health and Safety inspection was conducted on 11/05/2019 to review supplemental health and safety requirements. There were 38 children, ages 5 years-11 years, 3 staff providing direct care and supervision and additional support staff present during the inspection. The inspector reviewed 5 children's records, 5 staff records and 2 medications during the inspection. Children were observed in the following activities: having breakfast, during a fire drill, going on a nature walk and in free choice large motor play. Proper hand washing and table sanitizing procedures were also observed. The inspector reviewed the emergency supplies during the inspection. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and can be found on the Violation Notice. Description: Based on record review and discussion with the staff, the vendor failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The record for Child #3 contained documentation of a diagnosed food allergy; there was not a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction in the record for Child #3.

Plan of Correction: The form will be gotten from the parent.
