Comments:
The inspector conducted an unannounced mandated inspection from 10:30 a.m. until 12:30. There were 12 children in care with three staff present. One child was observed being picked up by their parent and one parent was interviewed. Children were observed at circle time, playing and washing hands. Staff records were reviewed at the main office. Five children's records were reviewed. Violations are identified on the violation notice. Please call your inspector at 276-698-9981 with any questions.
Based on observation, the center failed to maintain the center in a safe condition. Evidence: 1. The inspector observed a broken wheel and missing petals on the multi-red tyke play equipment on the playground.
Plan of Correction:
The director said that the piece of equipment would be removed.

Description:
Based on observation and measurement, the center did not meet the minimum safety standards for fall zones on the playground. Evidence: 1. The inspector measured the fall zone for the piece of gray climbing equipment and there was 5 feet of mulch; the requirement is 6 feet for climbing equipment on all sides. 2. The inspector measured the mulch on the playground around the slide and the climbing equipment, and it was 2-3 inches thick; the required depth is 6 inches. Some of the pieces of mulch were 4 inches long.
Plan of Correction:
Mulch will be added and redistributed and fluffed. The goal is to have rubber mulch.
