Comments:
An unannounced monitoring inspection was conducted on 10/23/2019 from 12:00 pm - 1:40 pm. At the time of the tour there were 67 children in care with 16 staff. The children in care ranged in age from infant - three years. Children were observed during lunch and nap. Toileting and hand washing was observed in the preschool classrooms.
Records were reviewed for ten children. Medication was reviewed in addition to documentation of children's injuries.
Violations were observed in three parts of the CDC standards and are listed on the violation notice. These violations were reviewed with the program director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that areas of the center shall be maintained in a safe condition.

Evidence:
1. An open bag of root beer flavored hard candy was observed in an accessible and unlocked desk drawer in room 107.
a. Hard candy is a choking hazard for children under the age of three years.
b. The children in classroom 107 are two years of age.

Plan of Correction: The hard candy was locked up during the inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children.

Evidence:
1. A container of sanitizing wipes was observed in an unlocked desk drawer in classroom 107.
a. The label on this product contained multiple warnings to indicate the contents as hazardous.
2. A small container of hand sanitizer was observed in an unlocked desk drawer in classroom 107.
a. The label on this product contained multiple warnings to indicate the contents as hazardous.

Plan of Correction: Both products were locked up during the inspection. staff will be reminded to maintained all hazardous chemicals in a locked place.

Standard #: 22VAC40-185-520-C
Description: Based on observation, the center failed to ensure that the container of diaper ointment shall be labeled with the child's name.

Evidence:
1. Diaper ointment for child 1 and child 2 was not labeled with the name of each child.
a. Both products were stored inside of a labeled bag however, the standard requires that the actual product is labeled with the name of the child.

Plan of Correction: Both ointments were labeled with the child's name during the inspection.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the center failed to ensure that good brought from home shall be labeled in a way that identifies the owner.

Evidence:
1. A container of breast milk, observed in the refrigerator in the infant room, was not labeled with the name of child 3
a. The container was dated to indicated when the milk was pumped.
b. Staff verbally stated that the breast milk was for child 3.

Plan of Correction: The container of breast milk was labeled with the name of child 3 during the inspection.
