A complaint investigation was conducted on 1/22/2020 regarding noncompliance with ratios and staff qualifications. Interviews were conducted, and documentation was reviewed. There was not a preponderance of evidence to support the allegations. However, three non-related violations are identified in the Violation Notice.

Based on record review, 3 of 4 staff records did not contain documentation of a current TB test at least every two years from the date of the first initial screening or testing.
Evidence: 1. Staff A's (date of hire: 11/4/2013) most recent documented TB test was dated 3/12/2016 (expired: 3/12/2018). 2. Staff C's (date of hire: 7/21/2014) most recent documented TB test was dated 10/29/2016 (expired: 10/29/2018). 3. Staff D's (date of hire: 8/29/2007) most recent documented TB test was dated 10/4/2013 (expired: 10/4/2016).

Support staff who need TB screening by allowing time during the work day for staff to complete test.

A written record has always been kept, however due our Wifi connection that is sometimes unreliable, there were records not saved to software. On 1/27/2020, we implemented handwritten sign in sheets.
