An investigation was initiated on 7/23/19 in response to a self reported incident on that same date regarding supervision and inappropriate discipline of a child by a staff person. An unannounced on-site inspection was conducted on 7/26/19 and interviews with staff were conducted. Documentation, records and video footage related to the incident were also reviewed. A follow-up visit to the facility was conducted on 8/2/19 and additional staff interviews were conducted. Based on the information gathered during the investigation, the self reported incident is determined to be not valid. There were no violations cited as a result of this investigation.
