Technical Assistance:
A discussion with management included placement of the crockpot and replacing the current crockpot with a cool touch crockpot. Additionally, adding a barrier to prevent the infants from being able to touch the crockpot.

Comments:
A complaint involving a child who sustained burns from a crockpot used to heat bottles was investigated on 09/29/2022 and 10/04/2022. The investigation included staff interviews, parent interviews, a review of staff records, a review of child records, and physical space inside the infant classroom. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on a review of documentation and staff interview, the center did not inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
Evidence:
There was no documentation available for review to show that an injury requiring outside medical treatment (IROMT) had been reported as required. Multiple staff stated they were not aware of the standard to report such injuries. The incident occurred on 09/16/2022 and as of 10/04/2022 no one had contacted the OCCHS office or Inspector.

Plan of Correction: An email was sent to the representative on the next business day following the incident to Ariele.Hayes on Sept. 19th regarding the crock pot. The email was provided along with the accident reports as required for review on 10/04 during the onsite review.

Description: Based on multiple staff interviews, it was determined that the center failed to ensure care and protection while supervising two children.
Evidence:
1. On 9/16/2022 at approximately 11:44 am, infant #1 was being held by Staff C, having their hands washed after an art project. The infant was burned by coming into contact with the crockpot used to heat bottles that is located next to the sink. Staff D notified Staff A at the time of injury.
2. On 09/16/2022 at approximately 1:30 pm, infant #2 was being held by Staff C, having their hands washed. The infant was burned by coming into contact with the crockpot used to heat bottles that is located next to the sink. Staff F notified Staff B of the injury.
3. Infant 1 & Infant 2 received burns across the top of their fingers from the crockpot.

Plan of Correction: Our team notified the family immediately via phone and provided accident reports at pick-up. It was determined that Staff C, a newly hired employee, did not disclose a medical issue during the hiring process, state background check process, CPR/First Aid Health Certification or on the hiring and orientation paperwork to our program or the state as required. Based on her failure to disclose, she was transitioned from the team following this incident.
