Comments:
This Inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 05/09/2020 and concluded on 05/09/202. The director was contacted by phone to initiate the inspection. There were no children present and no staff. The inspector emailed the provider/director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
Based on a review of available documentation, a Sworn Disclosure Statement was not signed prior to the first date of employment. Evidence: Staff A (Date of Employment (DOE): 02/18/2020) has a signed Sworn Disclosure Statement (SDS) signed 02/20/2020 on file.
Plan of Correction:
Sworn Disclosure Statements will be signed prior to the staff member's start date.
