Technical Assistance:
Technical assistance was provided regarding verifying proof of identification and the annual review of children's emergency contact information.

Comments:
An unannounced monitoring inspection was conducted today from 9:45am-2:25pm. There were 95 children directly supervised by 15 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 5 staff records, 10 children?s records, 4 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed coloring, learning to count, listening to a story, playing inside in the Boulevard and going down the slide, playing on the playground, and infant care including feeding and diapering was also observed. Lunch served today is a bean burrito, apple sauce, corn, and milk. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-160-C
Description: Based on record review, 1 of 5 staff records reviewed did not contain documentation of an updated tuberculosis screening or test within the past two years.

Evidence:
1. Staff #3's most recent tuberculosis screening on file was completed 9/20/2016.

Plan of Correction: On Monday the staff will have a screening done.

Standard #: 22VAC40-185-440-F
Description: Based on observation of the physical plant, children's rest mats could not be properly cleaned and sanitized weekly or before use by another child.

Evidence:
1. Multiple rest mats used by the children in care are torn exposing the foam cushioning preventing them from being properly cleaned and santized on a weekly basis.

Plan of Correction: I ordered new mats in October and they should be delivered soon.

Standard #: 22VAC40-185-510-N
Description: Based on medication and documentation review, not all medications were picked up by the parent within 14 days of authorization expiration and continued to be stored on site.

Evidence:
1. The medication authorization for Child #11 expired on 9/13/2018 and the medication was stored on site.

Plan of Correction: I gave the family the paper for the medication authorization to be renewed.

Standard #: 22VAC40-185-520-C
Description: Based on observation of the physical plant, a diaper ointment was not labeled with the child's name.

Evidence:
1. In the Infant classroom, there was a tube of A&D cream that was not labeled with a child's name.

Plan of Correction: The A&D ointment was discarded.
