Comments:
A monitoring inspection was conducted today between 10:30am and 11:30am. There were 3 children present with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision. The children were observed playing together. A total of 3 child records 1 caregiver, and 1 household members records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of household member records, background checks were not renewed every 5 years. Evidence: Household member #1 had documentation of an expired Sworn Disclosure Statement . Statement was dated 10/22/2017. Household member #1 did not have documentation of a completed criminal history check. Household member #1 did not have documentation of Central Registry results

All background checks will be completed. I will let my inspector know when background checks are completed.

Based on review of provider and household member records, individuals did not obtain additional tuberculin skin (TB) tests every two years. Evidence: Provider#1 had documentation of a TB test last obtained on (12/13/2019). Household Member #1 had documentation of a TB test last obtained on (12/13/2019).

We will get our TB tests done and put the results in our files.

Description:
Based on review of children?s records, the provider did not maintain an individual record for each child enrolled in care. Evidence: Child #1 had no record of emergency or authorized pick-up contacts and parent agreement. Child # 2 had no record of emergency or authorized pick-up contacts, name, address or telephone number of parents? place of employment, proof of identity and age, health examination. Child # 3 had no record of health examination
Plan of Correction:
I will contact the parents and get the required documents. I will review all my records to make sure they are complete.
