Comments:
An unannounced renewal inspection was conducted on 9/16/2022 from 11:40pm to 1:20pm. There were 2 children (7 points) in care during the inspection. The home, 1 staff record, 2 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely and eating lunch. The home was clean and organized. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on interview, the provider refused to permit immediate admission to the family day home to an authorized representative of the department while children were present in the home.
Evidence: 1.The provider was not in the home when the inspector arrived. Adult #1 was in the home and confirmed that there were two children taking a nap at the time the inspector arrived at the home. 2. At approximately 11:40am, the inspector asked Adult #1 to call the provider to let them know their inspector was at their home to conduct an inspection. 3. The provider stated that the inspector could not enter the home to conduct the inspection until she returns home. 4.The inspector was unable to enter the home for approximately 60 minutes, the provider arrived at the home at approximately 12:40pm.
Plan of Correction:
I had a doctor's appointment. I had to go after a visit from the hospital. My brother does not watch them. It was an emergency. I usually take the day off. My brother did not know her. With all that is going on in the world he was skeptical.

Description:
Based on record review, the provider did not maintain on file a signed statement from each parent, affirming receipt of the information to parents statement.
Evidence: The records of Child #1 and Child#2 did not contain a copy of the Information to Parents statement.

I had not been given any. The inspector gave me a copy and I have copied some and have been given to parents and signed.

Evidence: The records of Child #1 (DOE: 4/19/2021) and Child #2 ( DOE: 9/20/2021) did not contain a copy of the physical examination results for each child within 30 days after enrollment.

Description:
Based on observations, the provider did not meet all the criteria on the health and safety checklist at the time of the monitoring visit.
Evidence: 1. The caregiver did not wash Child #1's hands before the child was placed in a high chair to eat their lunch. 2. An operable flashlight was not available for review during the inspection. 3. The emergency evacuation drill log was not available for review during the inspection.
Plan of Correction:
1.He had just got up from a nap. I wash them when we have played outside. 2.It has been purchased. Already purchased corrected the next day. 3.It has been emailed to inspector.
