Comments:
An unannounced monitoring inspection was conducted Thursday, August 5, 2021 from approximately 2:00PM until 3:15 PM to determine compliance with the requirements for Voluntary Registration. Two children were in attendance being supervised by one caregiver. Based on the children?s age there was a total of four points. Children were observed reading books, singing songs, watching children?s programs on tablets, working puzzles, playing with toys and taking bathroom breaks. All areas in the home used by children were inspected. Administrative documentation was reviewed. Two children's records and one caregiver?s record were reviewed. Based on review of four children?s records, observations and interview, the provider did not meet all the criteria on the health and safety checklist at the time of the inspection.
Evidence: 1. Two electrical outlets located in the living room where the children were playing did not contain protective covers.

I have extra electrical outlet covers and I will put them in.

Description:
Based on review of vendor?s record and interview it was determined that one central registry checks was not repeated every five years. Evidence: The record for Vendor #1 contained a central registry check dated 03/24/2016.
Plan of Correction:
The licensing inspector will send the form that needs to be completed and I will send it off.
