Comments:
A renewal inspection was initiated and concluded on June 28, 2022 to determine compliance with the requirements for Voluntary Registration. The licensing inspector was on site from approximately 10:30am until 12:30pm. The provider assisted the licensing inspector during the inspection. There were zero children in care and one caregiver present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication. The inspector reviewed four children?s records and one provider?s record.
Information gathered during the inspection determines non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on review of one provider's record and interview, the provider did not ensure background checks were obtained at least every five years.
Evidence: The record for Provider #1 contained a central registry finding dated 03/24/2016. Provider #1 stated she mailed the packet to the Central Regional Office for processing instead of Office of Background Investigations..
Plan of Correction:
I thought I was to submit the entire renewal packet the Central Office for processing. I did not realize a different department processed background checks. I will mail the central registry form to the correct office along with the fee.

Description:
Based on review of four children's records and interview, the provider did not obtain the required information for four children.
Evidence: 1. The records for Child #1, Child #2, Child #3 and Child #4 did not contain written agreement made between the family day provider and the natural parent, guardian, or other responsible person for each child in care. The agreement may cover hours of care per day, week, or month; cost of care per day, week, or month; frequency and amount of payment per day, week, or month; and any special services to be provided by either party to the agreement. 2. The record for Child #1 did not contain results of the health examination and up-to-date immunization records. 3. The record for Child #4 did not contain results of the health examination.
Plan of Correction:
I will give the paperwork for families to complete and add to the record.
