Comments:
An unannounced monitoring inspection was initiated and concluded on March 15, 2023, to determine compliance with the requirements for Voluntary Registration. The licensing inspector was on site from approximately 10:30am until 12:45pm. The provider assisted the licensing inspector during the inspection. There was one child in care, 15 months of age, with one provider present. The licensing inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication. The inspector reviewed four children?s records and one provider?s record.
Information gathered during the inspection determines non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of four children?s records, the provider did not maintain the required information in the record for each child enrolled in care.
Evidence: 1. The record for Child #1 (first date of attendance 12/04/2022) did not include name, address, and telephone number of one or more persons designated by the parent or parents to be called in case of emergency when a parent cannot be reached during the hours the child is in care. 2. The records for Child #1 (first date of attendance 12/04/2022), Child #2 (first date of attendance 06/01/2022), Child #3 (first date of attendance 06/01/2022) and Child #4 (first date of attendance 05/01/2022) did not include any written agreement made between the family day provider and the natural parent, guardian, or other responsible person for each child in care. The agreement may cover hours of care per day, week, or month; cost of care per day, week, or month; frequency and amount of payment per day, week, or month; and any special services to be provided by either party to the agreement.

Based on interviews, observations, and review of four children?s records, the provider did not meet the criteria on the health and safety checklist at the time of the monitoring visit.
Evidence: 1. The provider stated she needs to complete evacuation drills monthly to the point of exit from the home. 2. The record for Child #1 (first date of attendance 12/04/2022) did not include results of the health examination. 3. The record for Child #1 (first date of attendance 12/04/2022) did not include results of the health examination. 4. The records for Child #1 (first date of attendance 12/04/2022), Child #2 (first date of attendance 06/01/2022), Child #3 (first date of attendance 06/01/2022) and Child #4 (first date of attendance 05/01/2022) did not include written notification of the fact that the family day home business is covered by liability insurance, along with amount, or that the business is not covered by liability insurance to the parents or guardians of all enrolled children. Signed acknowledgement of written notification is maintained on file for each child during the child?s attendance and for 12 months after the child?s last day of attendance.
