Comments:
An unannounced monitoring inspection was conducted between 9:00am and 12:30pm. Upon arrival there were 22 children with 5 staff present. A sample of 1 child and 3 staff records were reviewed. Children were observed playing in centers, watching Elmo on television, during lunch and nap.
Based on a record review the center did not ensure all required information was kept for each staff person.
Evidence:
Staff #3 stated her job title is a program leader and assistant director. Staff #3's file documents her job position as a teacher assistant. Staff #3 stated she was hired as a teacher assistant but was promoted. The promotion was not documented in the file.
Plan of Correction:
The following corrective action was provided by the assistant director during the exit interview:
The director will document staff #3's promotion to accurately reflect her current title.
