An unannounced monitoring inspection was conducted between 8:00am and 12:00pm. A sample of 6 children and 4 staff records were reviewed. Children were observed on the playground, circle time and playing with staff. Infants were observed sleeping in cribs, being fed and playing on the floor.
Based on a record review the center did not ensure each staff member had a negative tuberculosis screening no later than 21 days after employment. Evidence:1. Staff #1 (hired 07/22/19) did not have documentation of a tuberculosis screening. The director confirmed the staff member did not have one and was sent that day to obtain it.
Plan of Correction:
The following corrective action was provided by the director during the exit interview: Staff #1 was sent that day to obtain a tuberculosis screening.

Description:
Based on a record review the center did not ensure all staff had written information demonstrating their qualifications for their job positions. Evidence: Staff #1 (hired 07/22/19) was listed in the file as a program leader. The director confirmed that was her job position. The center did not have documentation that Staff #1 met the qualifications of a program leader. The director confirmed that she had not seen a transcript of any other form of written information that confirms her qualifications.
Plan of Correction:
The following corrective action was provided by the director during the exit interview: Staff #1 had qualifications listed on the resume, however a transcript was not received. The director will follow-up with staff #1 to obtain a copy of her qualifications for the job position as program leader
