Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated and concluded on 11/30/20. The provider was contacted by telephone to initiate the inspection. There were 9 children present (20 points), including the caregiver's child, and 2 caregivers. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 caregiver records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. For any questions related to this inspection, please contact me at 571-835-4718 or mahrukh.aziz@dss.virginia.gov.
Based on staff interview, the family day home provider did not obtain central registry findings for a household member becoming 14 years old, within 30 days of reaching that age. Evidence: 1. The provider stated that Household Member (HHM) # 1 turned 14 years old on 4/30/20. 2. The provider stated that the Virginia Central Registry form for HHM # 1 had been mailed, but was returned due to missing information. The form was not resent.
Plan of Correction:
The form will be resent by the end of this week. A follow up will be conducted with the inspector as soon as the results are obtained.
