Technical Assistance:
The provider was given written information regarding substitute providers, 8VAC20-800: 10 (definition); 70-A-15; 140-C and D; 150-E; 650-C via email.
Comments:
A monitoring inspection was conducted on 5/3/2023 from 11:30 am to 1:05 pm. Nine children, ages 6 months to 4 years (total of 24 points) were in care with two caregivers. The Licensing Inspector reviewed three children?s, two caregiver records, and other program records. The children were observed in quiet play and napping.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review, the provider failed to obtain documentation of a physical examination for a child within 30 days of after the first day of attendance.
EVIDENCE: Child 3 began attendance on 11/30/2022 and the record does not have documentation of a physical examination.

Parent was called during the inspection. They will get the forms from the child's doctor.

Description:
Based on observation, the provider failed to have the water temperature maintained within a range of 105 F to 120 F.
EVIDENCE: A reading with a digital thermometer showed the water temperature at the sink children use was 123 Fahrenheit.
Plan of Correction:
Upon checking the water to adjust it, the temperature read 110. So Provider thought the thermometer she was using was not reading properly. The provider purchased a new thermometer and the reading was 110.3. She adjusted the temperature anyway, to read at 106 degrees.
