Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated and concluded on 6/14/2021. The provider was contacted by telephone to initiate the inspection. There were (2) resident children present and (1) caregiver. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records and (2) caregiver/household records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-210-A
Description: Based on interview and record review, the caregiver did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1. Caregiver 1 had documentation of 3 hours of training and stated she completed 2 more hours, but did not complete all 16 hours of training.

Plan of Correction: I will complete the training hours by tonight.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous substances were not stored away inaccessible to children.
Evidence:
1. 11 cleaning supplies were located under the kitchen sink in an unlocked cabinet.

Plan of Correction: I found the child safety lock and put it on the cabinet.
