Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which play groups, activities, group time, outdoor time, personal hygiene, and lunch time were observed. Children were observed; and staff and children files and other documentation were reviewed. Forty-nine (49) children, aged 8 weeks-4 years, were found to be in care with 10 staff, kitchen staff, and administrative staff. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 9:45 am and concluded at 12 noon.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observations the facility failed to maintain compliance with this standard as required relating to locking of areas with hazardous substances.

Evidence:

The kitchen door and door to the staff bathroom area were not locked and there were hazardous substances accessible to the children in care. The doors were not tightly shut, and opened with a slight push.

Plan of Correction: The doors were immediately shut tightly, allowing for the child proof handle to engage and no longer easily opened. Staff will be reminded that the doors must firmly close and latch to protect children from gaining access.

Standard #: 22VAC40-185-510-A
Description: Based on review of medications the facility failed to maintain compliance with this standard as required relating to authorization to administer medication.

Evidence:
The permission to administer form had expired for child A, Epi-Pen, as pf 8/1/19. The medication was still in the active medication box.
The permission to administer form had expired for child B, Pro-Air, as of 7/3/19. The medication was still in the active medication box.

Plan of Correction: The parents were immediately notified as to the need for an updated permission form in order for the medications to be administered or kept on site. Medications will continue to be checked and any updates needed will occur.

Description: Based on review of medications the facility failed to maintain compliance with this standard as required relating to labeling of children medication.

Evidence:
The medication for child D, Avi-Q, did not have the required pharmacy label attached or available.

Plan of Correction: The parent was immediately contacted and a pharmacy label was requested. Medications will continue to be reviewed and any updates made accordingly.
