An announced non-mandated record review inspection for the YMCA Glade Hill Elementary After School Program was completed on 08/20/2019. The inspector reviewed 3 staff records during the inspection today. The inspector arrived at 12:10 p.m. and departed at 1:50 p.m. Thank you for your time today. Based on record review and discussion with the staff, the facility failed to ensure that staff records contain all of the elements as required by the standards.
Evidence:
1. The record for Staff #1 did not contain the health information as required by 22VAC40-185-160.
2. The record for Staff #2 did not contain written information to demonstrate that the individual possesses the experience required by the job position. The record for Staff #2 had Program Lead as the documented job position.
Plan of Correction:
The Child Care Coordinator will get the missing information.
