Comments:
An unannounced monitoring inspection was completed on 09/12/2019. There were 18 children, ages 5 years-10 years, and 2 staff present during the inspection. The inspector reviewed 5 children's records, the on-site requirements for 2 staff records and 1 medication during the inspection. Children were observed in the following activities: arriving to the program, during homework time, having an after school snack and in large motor play in the gym. The inspector reviewed the emergency supplies during the inspection. The inspector reminded the staff of the upcoming expiration of the fire inspection. The inspector arrived for the inspection at 2:20 p.m. and departed at 4:30 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.
Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled contain all of the elements as required by the standards.
Evidence: The record for Child #1 did not contain the following information: the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (there was only one person designated and the address for that designated person was incomplete) and the previous child day care and schools attending by the child.
Plan of Correction:
Site staff will get the information from the parent.
