22.1 BACKGROUND CHECKS CODE; CARBON MONOXIDE

Comments:
An unannounced renewal inspection was conducted from 10:30 am - 11:15 am on 03/22/2023 and from 1:45 pm - 3:00 pm on 03/30/2023. There were 27 children in care being supervised by 5 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-770-60-C-2
Description: Based on review of three staff records it was determined that not all records contained documentation of central registry checks within the first 30 days of employment. Evidence: The records for staff A and Staff B (start date: 02/13/2023) did not have documentation of a central registry check.

Description: Based on observation inside and outside of the facility it was determined that not all areas were maintained in safe and operable condition. Evidence: 1.) There are hanging cords accessible from the bottle warmer and telephone that are on the toddler side of the counter between the toddler room and infant room. 2.) Two push toys were observed placed on top of a low toy shelf. 3.) There are nap cots being used to block the entrance from the infant and toddler room and the two's/three's room posing a tripping hazard. 4.) The back portion of the playground fence is falling down. 5.) There are protruding nails on the second wood plank to the right of the entrance to the playground. 6.) There are sharp lawn tools in an unlocked storage container on the playground.

Description: Based on observation it was determined that not all hazardous substances were kept locked. Evidence: 1.) Six cans of spray paint with instructions to keep out of reach of children were observed in an unlocked storage container on the playground. 2.) A bottle of baby oil with instructions to keep out of reach of children was observed unlocked in the 4's/5's classroom.

Description: Based on observation and interview it was determined that pillows were not prohibited for children under two years of age. Evidence: A boppy pillow was observed in the crib of child D (Age 4 Months). It was reported by staff that child D utilizes the pillow for sleep.

Description: Based on observation it was determine that over the counter skin products were kept beyond their expiration date. Evidence: The diaper ointment for child A had an expiration date of 07/2022. The diaper ointment for child B had an expiration date of 01/2023. The diaper ointment for child C had an expiration date of 02/2023.
