Comments:
An unannounced monitoring inspection was initiated and concluded on June 8, 2023. This monitoring inspection was a follow up to the Intensive Plan of Correction previously developed by the center There were 90 children in care with 16 staff and administrators. Classroom observations were made, and children were observed in teacher
and self-directed activities. Violations were cited in the areas of Administration, Special Care Provisions and Emergencies and Background checks. These violations appear on the violation notice and were discussed with the program administrator during the exit interview at the conclusion of the inspection.

Description: Based on record review and interview the center failed to ensure that background checks are completed as required.
Evidence:
1 ? The record for Staff #5 contains fingerprint background check results dated November 30, 2022. However, the documented date of hire is November 29, 2022.
2 ? Staff #5 was present providing direct care to children enrolled in the program.
3 ? Staff #1 confirmed that the results of the fingerprint background check were received the date of hire for Staff #5.

Description: Based on record review and interview the center failed to ensure that tuberculosis screenings are obtained prior to staff coming in contact with children.
Evidence:
1 ? The record for Staff #3 contained tuberculosis results dated January 22, 2023. However, the documented date of hire, including contact with children, is January 12, 2023.
2 ? The record for Staff #5 contained tuberculosis results dated March 11, 2023. However, the documented date of hire, including contact with children, is November 29, 2023.
3 ? Staff #1 confirmed that the tuberculosis result information was obtained after Staff #3 and #5 had been in contact with children in care.

Description: Based on observations and interviews the center failed to confidentially maintain a current written list of all children's allergies, sensitivities and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. Additionally, The list was not dated and kept in each room or area where the children are present.
Evidence:
1 ? The allergy list posted in the three-year-old room is posted on the wall and visible to everyone in the classroom.
2 ? The information on the posted allergy list is not dated.
3 ? The information available to staff in the classroom does not include the allergy plan for Child #1.
4 ? The room where children are served lunch does not have, accessible to staff, information regarding children?s allergies or allergy plans.

Description: Based on observations and interviews the center failed to ensure that disposable diapers are disposed of in a leakproof or plastic-lined storage system that is foot-operated.
Evidence:
1 ? The diaper pail in the two-year-old room requires the user to lift the lid with their hand in order to dispose of a soiled diaper.
2 ? Staff present in the room confirm that the disposable diaper pail in the two-year-old bathroom required them to lift the lid with their hand to throw away soiled diapers.
