Comments:
Licensing inspector arrived at 10:45 am and departed at 12:45 pm to conduct a licensing inspection and a Subsidy Health and Safety Inspection. At the time of the tour there were 24 children with 6 staff members. An additional staff person were on site for administrative and support purposes. Children were observed preparing for nap, eating lunch, freely playing and watching a movie. Sample size six staff records and five children's records.
Based on record review and staff interview, the licensee did not ensure the staff records included documentation of two references.
Evidence: Staff #2's record was missing documentation of two references.
Plan of Correction:
The director will try to locate the missing documents or will redo the references.

Description:
Based on observation and staff interview, the licensee did not ensure areas and equipment of the center inside, was maintained in clean, safe and operable condition.
Evidence: In the boys restroom, there was a board approximatley 3 feet tall with two protruding nails at the end.
Plan of Correction:
The board was removed form the bathroom and bathrooms will be checked daily.
