Comments:
An unannounced renewal inspection was conducted on February 14, 2023 from approximately 10:30 am to 11:45 am. There were 20 children in attendance and a total of 5 staff present. 2 staff records and 3 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed having a Valentine?s Day party, reading books, and playing outside while the licensing inspector was on-site.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 02/23/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).
Based on records review, the center failed to obtain documentation that each child has received the immunizations required before the child can attend the center.
Evidence:
1) The record for child #2, date of enrollment, 02/06/23, did not contain documentation of immunizations. 2) The record for child #1, date of enrollment, 02/6/23, did not contain documentation of immunizations.
Plan of Correction:
Per the Director "both students now have complete immunizations records. Additionally, SSA will no longer accept appointment cards in place of records."

Description:
Based on records review, and interview, the center failed to follow the terms of the current license issued by the department. Evidence: The record for child #2, date of enrollment 02/06/2023, determined that child #2 is 2 years 5 months. The term of the current license is for the ages of 2 years 6 months through 12 years. When interviewed, the Assistant Director confirmed that child #2 was not 2 years 6 months of age.
Plan of Correction:
Per the Director "student will be 2 years and six months on 3/17. Director will not permit admittance before 2 years 6 months moving forward."
