Technical Assistance:
Poison ivy trailing across the playground fence was discussed with the program director.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 11:50 AM and departed at 2:15 PM. Lunch service and nap time were observed. Five staff records and a sample size of five children's records were reviewed. It was reported that no medication is administered to the children. In jury records were reviewed.

An exit meeting was conducted with the program director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based upon observation and staff interview, the facility has not ensured that children under age ten years are always within actual sight and sound supervision.
Evidence:
1. Two infants and a toddler were in low infant seats in the middle preschool classroom. The inspector walked out of the older preschool room and there was no staff person with the children. Staff 1 was in the kitchen and saw that the teacher had stepped out of the room.
2. Staff 2, who was on duty with the two infants and a toddler in the middle preschool classroom was observed in the toddler room on two additional times. Staff 1 observed this with the inspector.
3. Staff 1 verified that staff 2 left the infant grouping momentarily unattended in the middle preschool classroom.

Plan of Correction: The facility responded with the following:
Staff training will continue so that all staff understand that even though the two classrooms open into each other, they are separate rooms and must have staff directly within sight and sound supervision.

Description: Based upon observation and staff interview, the facility has not ensured that the ratio applicable to the youngest child in each grouping of children is followed:
Evidence:
1. Upon arrival of the inspector at 11:50 AM, there were six total children, ages 18 months through three years in toddler seats and at the table in the toddler classroom with one staff person (staff 5).
2. At 11:55 AM, staff 1 arrived, acknowledged the needed staff:child ratio of one staff to five children and adjusted ratio by adding herself to the group.

Plan of Correction: The facility responded with the following:
The sixth child arrived while staff 1 was running a quick errand. One staff person was out sick. We are working on hiring an additional/floater staff person. In the future, staff will required the parent to remain with their child until an additional staff person returns to the facility.
