Technical Assistance:
Discussed children's records, medication authorization forms, out-of-state background checks and CPR/First Aid certification.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 03/02/2021 and concluded on 03/17/2021. The director was contacted by telephone to initiate the inspection. There were 24 children present and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review, it was determined that the center did not ensure that employees that had lived out of the state of VA within the last 5 years have in their files the result of an out-of-state Sex Offender Registry search request, an out-of-state Criminal History Name Check and an out-of-state Central Registry search request. Evidence: 1) The record for Staff #3 did not contain documentation of an out-of-state Sex Offender Registry search request and an out-of-state Criminal History Name Check. The record for Agent #1 did not contain documentation of a Central Registry search request and an out-of-state Sex Offender Registry search request. Staff #3 and Agent #1 had declared in their sworn statement having lived out of the state of VA within the last 5 years before employment.
Plan of Correction:
I will follow up with department of IL for an update this week. The Sex Offender search for Agent #1 was completed on 03/17/2021. Will request the Central Registry search ASAP.
