Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 10:00 AM and departed at 12:30 PM. Eighteen children were in care at the start of the inspection. A sample size of five staff records and five children's records were reviewed. Medication administration was reviewed. No children's injuries were reported.

An exit meeting was discussed with the assistant director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based upon review of five children's records, the facility has not ensured that each child's record includes the names, addresses and telephone numbers for two designated persons to be contacted in an emergency when a parent cannot be reached.
Evidence:
1. The record provided for child 1 does not have a complete street address for one of the two emergency contact persons.
2. The record provided for child 2 does not include street addresses for the two emergency contact persons.

Plan of Correction: The facility responded with the following:
Both parents will be notified today that complete street addresses must be entered in each record.

Standard #: 22VAC40-185-70-A
Description: Based upon review of five staff records, the facility has not ensured that each staff record includes written information to demonstrate that the individual possesses the experience required by the job position.
Evidence:
The record provided for staff 1 indicated a hire date of 6/4/2019 as a program leader. The record did not include written verification/documentation of at least six months programmatic experience.

Plan of Correction: The facility responded with the following:
The previous experience of staff 1 will be verified and documented.

Standard #: 22VAC40-185-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by children.
Evidence:
1. The children pass through a storage area to access the back playground. There were cans of paint, a gallon bottle of bleach and laundry products on the floor of the storage area. All were marked with hazard warnings.
2. There was a container of sanitizing wipes and two pump bottles of hand sanitizer in unlocked drawers in a filing cabinet in the pre-K classroom.

Plan of Correction: The facility responded with the following:
1. The cleaning products in the storage area were moved to the locked cabinet during the inspection.
2. The products in the pre-K classroom were moved to a locked location during the inspection. Staff were reminded that all hazardous substances must be kept in locked locations.

Standard #: 22VAC40-185-500-B
Description: Based upon observation, the facility has not ensured that the diapering area is equipped with a diaper disposal system with a lid.
Evidence:
Although the diaper disposal system in the infant room had a lid, the lid was not attached to the disposal can.

Plan of Correction: The facility responded with the following:
The lid was attached to the disposal can during the inspection. Staff were reminded that the lid must be attached and working at all times children are in care.

Standard #: 22VAC40-185-510-A
Description: Based upon review of the medication stored at the facility and accompanying authorizations, the facility has not followed their medication policy requiring current parental authorizations for medication.
Evidence:
1. There was a prescription container of albuterol for child 1. There was no parent authorization for the medication and the physician authorization expired 4/26/2019.
2. There was a container of acetaminophen for child 2. The parent authorization form expired on 7/29/2019.
3. According to staff 6,neither of the medication were administered to child 1 or child 2.

Plan of Correction: The facility responded with the following:
The medications will be returned to the parents. Written parent authorization will be required prior to any medication being accepted in the facility. A tracking method will be established to ensure required authorizations are obtained.
