Comments:
An unannounced monitoring inspection was conducted on October 28 between 11:00am and 12:15pm. There were 8 children in the direct care of a provider and an assistant. Children were observed completing a candy corn art project, reading books, washing hands, drinking a bottle, and laying down for naps. The family day home day care is clean and organized. Based on record review, 1 of 5 children's records reviewed did not have documentation of additional immunizations every 6 months for children under the age of 2 years.
Evidence: Child # 2 record last documented immunization is dated 10/01/2018

Based on record review, 1 of 3 household records reviewed did not contain documentation that each caregiver and household member submit a current report of tuberculosis screening every two years from the date of the first screening.
