An unannounced monitoring inspection was completed on 10/10/2019. There were 24 children, ages 3 years-12 years, 3 staff providing direct care and supervision and additional support staff present during the inspection. The inspector reviewed 5 children's records and 3 staff records during the inspection. Staff stated that medication is not administered at the extended day program. Children were observed in the following activities: during rest time, departing from the end of school and arriving for after school, and during homework time. The inspector reviewed the emergency supplies during the inspection. The inspector discussed medication requirements with the staff. Based on record review and discussion with the staff, the facility failed to ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.
Evidence: The record for Child #2 did not contain documentation of a physical examination; the documented start date for Child #2 was 09/03/2019,

The physical was faxed to the facility during the inspection.
